Patient instruction during visit

Dear patient and respected companions: We wish you health and wellness and after welcome, we would like to thank you for choosing the "Third of Shaban" hospital as a hospitalization center. Please read the contents of the website carefully before visiting the hospital.

Upon reception:

  1. Upon entering, the patient is given a bracelet to identify the specifications, it accompanies him during his stay in the hospital and until he leaves it. The information on this bracelet include: the patient's name, family name, the entry code, and patient’s date of birth.
  2. To accept patients who have not reached the legal age and require surgery, a parent or legal guardian is required to complete the consent form, for women, the husband or the father must be.
  3. Patients who are covered by the supplementary insurance (except for Iran, Dana and Asia insurance companies) are required to submit a referral request not later than 24 hours at admission department, and that after entering, otherwise, hospital treatment costs are not guaranteed. For further assistance, refer to the department secretary.
  4. The hospital's reception department matches the patient’s insurance card with the patient himself, therefore, do not present another person’s insurance card instead of the patient’s card.
  5. All valuables are the responsibility of the patient himself during his stay in the hospital, please turn your valuables into those concerned,( If cash or jewelry is present, you can put it as a deposit with the hospital cashier and receive its receipt in the presence of the receptionist and protection officer.
  6. Observance and preservation of Islamic morals is mandatory for patients and companions. It is forbidden to have a male companion in the women's section, even in private rooms.
  7. In private rooms, one companion can be present for the patient, provided that he receives an accompanying card from the hospital reception office, it is also possible for a same gender companion to be present in the other rooms, provided that it is by written order from the attending physician, and due to the available space, therefore, this hospital is not concerned with providing amenities for companions in non-private rooms, and for companion’s costs, the receptionist will be concerned with answering the auditors regarding this.
  8. In all pediatric rooms, only one woman (preferably the mother) is mandatory.

Upon admission to hospital:

 

  1. After the admission process, a protocol nurse at the hospital will direct you to the relevant ward and room, in addition to handing in a hygienic bag, it will provide you with the necessary instructions.
  2. During your stay in the hospital, you will undergo all necessary medical and clinical procedures, if any diagnostic procedure outside the hospital is required, the supervisor carries out the necessary instructions and coordinates the transfer, and ....
  3. The time for making phone calls to patients is from 7 AM to 9 PM. You can contact your patient directly by calling the hospital number (+98-21-33129111-9) then the patient's room number.
  4. Some wards have a toll-free phone line to make calls outside the hospital or between two different wards. Use the phone card to call a cell phone or another city.
  5. Patient visiting hours range from 3 pm to 5 pm in the first six months of the year, and from 2 pm to 4 pm in the second six months of the year.
  6. Visiting patients in the Intensive Care Units (ICU and CCU) is restricted to one person only.
  7. Visits are prohibited for children under the age of 12, according to the instructions of the Ministry of Health.
  8. Due to the fact that this hospital is suitable for children, it is possible to follow up the breastfeeding process according to the opinion of the mother’s physician.
  9. In the event that a patient requests a private nurse, the patient's companion can submit this request to the Nursing Office.
  10. The necessary training will be provided to you orally and in writing (via brochure) by the attending physician and the nursing staff of the department.
  11. Meals hours for patients and companions: breakfast (6 am), lunch (12 noon), dinner (18:30 pm).
  12. The hospital concierge will respond to your request by providing the boiling water. (On two shifts, at 10:00 and 15:00).
  13. Please refrain from paying any money to hospital staff, and if you are asked by them, inform the hospital administration or the relevant administrative officer.
  14. The clinical supervisor monitors patients every shift, in case of any problems, please inform him to solve it. As that the administrator about the patient questionnaire will ask the patient, during his stay in the hospital, about the level of satisfaction with the services provided in the hospital (Nursing, treatment, well-being, health, etc.). If there is any criticism or suggestion about how services should be provided in this hospital, you can proceed with one of the following procedures:

At exit from the hospital:

  1. Upon exit from the hospital, receive all medical services documents inside and outside the hospital from the ward staff, in addition to a copy of the case file summary, prescription medication with prescription, a patient education sheet after exit and the specified time for the next visit to the clinic, and you must sign the book.
  2. Make sure to bring the deposit receipt.
  3. The exit of obstetric patients starts from 12:00 noon onwards.
  4. During the stay in the hospital or after the exit of obstetric patients, the father of the child must go to the birth certificate room, bringing with him the ID (or passport), the parents’ national card and the birth card of the child, until 2:00 in the afternoon as a maximum.
  5. The birth certificate is issued every day except for holidays from 12:00 to 14:00.
